Registering as a New Patient

We accept patients from the following postcodes only:

G44 3

G46 6

G76 0

To register as a new patient you will be required to complete a new patient registration form and a practice questionnaire.  Both can be downloaded, completed and brought to the practice along with proof of address.

If you bring along photographic ID, we'll be able to register you for our online service too.
